Initial Paperwork
For your first treatment you will be emailed our online clinic forms. If you require paper forms please arrive 20 minutes early. For the health of our chemically sensitive patients, please refrain from wearing body lotions, perfumes or colognes. Remember to ground your body by eating a light meal before your treatment and if receiving a group chair treatment wear loose clothing. We have a 24 hour advanced notice cancelation policy fee of $25 for community session and $45 for private table acupuncture. Many patients want weekly treatments please let us know in advance if you have an emergency or need to cancel/ reschedule. Thank you!
